Please read the following information

1. BEFORE STARTING THE TREATMENT I HAVE BEEN INFORMED ABOUT THE METHOD TO BE USED.

2. I HAVE DISCUSSED THE LOOK I WISH TO OBTAIN WITH THE ARTSIT AND I AM COMPLETELY SATISFIED.

3. I HAVE DISCUSSED SIZE COLOUR AND WE HAVE ARRIVED AT A DECISION TO MY TOTAL SATISFACTION.

4. I FULLY UNDERSTAND THAT DURING OR AFTER THE TREATMENT THE SKIN MAY SWELL OR REDDEN, THIS WILL SUBSIDE WITHIN THE NEXT 1-3 HOURS

5. FOR TWO WEEKS AFTER TREATMENT YOU SHOULD USE ANTI BACTERIAL SOAP AVOID COSMETICS ON THE AREA, AND DAB NOT RUB DRY.

6. I AM IN GOOD HEALTH AND FREE OF ANY ALCOHOL AND DRUGS, I'M NOT DIABETIC OR TAKING ANY BLOOD THINNING MEDICINES, IF SO PLEASE NOTE LATER ON THE THIS FORM AND MAKE YOUR ARTIST AWARE.

7. I AM NOT SUFFERING FROM ANY INFECTIOUS DISEASES.

8. I AM NOT GOING ABROAD OR ON A SUNBED FOR 2 WEEKS AFTER MY TATTOO. understand that if i do this may effect my tattoo and i accept liability for poor aftercare. results)

9. I UNDERSTAND THE AFTERCARE ADVICE WILL BE GIVEN TO ME BY MY ARTIST, WHICH WILL BE FULLY EXPLAINED DURING OR AFTER THE TATTOO IS COMPLETE. I AGREE TO FOLLOW THIS ADVICE. "understand that if I don't adhere to the aftercare guidelines given to me by my artist, I accept liability for poor aftercare results.

10. IF YOU HAVE ANY PROBLEMS OR QUERIES ABOUT YOUR AFTERCARE OR YOUR TATTOO DURING THE HEALING PROCESS, PLEASE CONTACT YOUR ARTIST AS SOON AS POSSIBLE TO ANSWER ANY QUERIES YOU MAY HAVE.

11. I AGREE THAT ALTHOUGH CLOSE RELATIVES OR FRIENDS MAY OFFER ADVICE FOR MY TATTOO AFTERCARE, I WILL FOLLOW THE AFTERCARE GIVEN TO ME, BY MY ARTIST SPECIFICALLY.

12. I UNDERSTAND IF I FOLLOW INCORRECT OR ALTERNATIVE AFTERCARE ADVICE. NOT INSTRUCTED TO ME BY MY ARTIST, THE TATTOO MAY HEAL POORLY AND/ OR WITH ADVERSE EFFECTS. I UNDERSTAND THAT IF I USE ANY AFTERCARE PRODUCTS, NOT SPECIFICALLY RECOMMENDED BY MY ARTIST, THE TATTOO MAY HEAL POORLY AND/ OR WITH ADVERSE EFFECTS.

13. I AGREE AND UNDERSTAND NOT TO TAKE RUBBISH, UP CLOSE PHOTOS WHILST BLOOD/INK ETC IS PRESENT SO THAT THE ARTIST CAN CLEAN IT DOWN FIRST AND TO ASK ABOUT SHARING TAGGING ON SOCIAL MEDIA ETC)

*I AGREE TO ASK PERMISSION AND MAKE MY ARTIST AWARE. BEFORE SHARING OR TAKING PERSONAL PHOTOS OF THE ARTIST DURING THE TATTOO BEFORE POSTING.

*I UNDERSTAND THAT MY TATTOO. IF NEEDED. MAY BE ELIGIBLE FOR A TOUCH UP FREE OF CHARGE ONCE IT HAS FULLY HEALED. THIS WILL BE RECOMMENDED TO ME BY MY ARTIST AND DISCUSSED WITH ME. HOWEVER, IF THE ARTIST DEEMS THAT THE CORRECT TATTOO AFTERCARE HASN'T BEEN FOLLOWED LEADING TO POORLY HEALED OR ADVERSELY AFFECTED FINAL RESULTS. THEY HAVE THE RIGHT TO REFUSE THIS TOUCH FREE OF CHARGE.
